It is doctor’s perception that matters to evaluate the situation. Hence, it is expected that doctor participate and provide the information based on their understanding considering patient’s condition and uses of the drugs.

Therefore, it is doctor’s responsibility to provide the data in given format. Please note at least 3 patient’s evaluation (data) is must in each form while minimum 5 patients evaluation is required to process 100% honorarium. In case of less than 3 patient data entry form will not be eligible for any honorarium and in case of less than 5 patients data entry honorarium will be calculated on pro rata basis. For example if total honorarium is Rs. 850, 100% would be processed for 5 or more patient data entry, while in case of 3 patients data entry Rs. 480 would be processed.
